Compiling a list of the scariest antagonists in video games is difficult because there are so many to choose from. Through the holiday season, the survival horror genre has made a triumphant return with monsters threatening to rend us apart in the Dead Space remake, The Callisto Protocol, the Silent Hill 2 remake, and the redesigned Resident Evil 4 game. The unsettling ambiance, the narrow corridors, the claustrophobic camera angles, the blinking lights (or lack of lights), the foreboding footsteps, and the skittering of unseen enemies — what’s not to be horrified of?
For a creature to make this list, being frightening is not enough. It must leave behind such severe psychological trauma that it spawns nightmares for years. You know, the type of monster that scars you so profoundly that you forget about it until something (such as this list) brings it up again.

6. Lisa (P.T.)
P.T. may have been a brief demo, but it lives on as a Hideo Kojima legend. (I will never remove it from my PlayStation 3) Lisa’s twitching head, bloody dress, missing eye, and constant smile make her a genuinely terrifying apparition. Also, there is nothing you can do about her. If you encounter her, she will terrify, possess, or off you on the spot.
5. The Twin Victims (Silent Hill 4: The Room)
The Twin Victims are arguably the most terrifying Silent Hill creature. This seven-foot-tall monster, also known as Doublehead, appears out of nowhere and simply points at you. Two eerie infant heads accuse you from a distance.
4. Kirby (Kirby, Super Smash Bros.)
Okay, just contemplate this one. Kirby is not a villain, but he is a monster — a happy, furious pink blob with the sucking power of a small cyclone that can consume anything and everything. In addition, Kirby is capable of stealing your essence (?), spitting you out, and duplicating you. Yeah, no thank you. Please keep clear.
3. Pyramid Head (Silent Hill 2)
Pyramid Head embodies the guilt and torment brewing within Silent Hill 2’s protagonist, James Sunderland, as it lumbers with the slow but inexorable pace of death itself. As a hulking executioner, he conceals his face beneath his signature helm and drags a greatsword across the floor. Pyramid Head is a reminder that the most terrifying creatures are the ones you create yourself.
2. Giygas (Earthbound / Mother 2)
Giygas embodies evil despite having no discernible form. Your party understands little about it throughout the entirety of the game, but you know the entire time that it is the game’s final boss. Once you confront its formless body, it transforms into a screen of red wave distortion that is so alien that you cannot comprehend its attacks. Some believe that this boss conflict is a metaphor for an alien psychic abortion. Whether or not this is true, Giygas is a cosmic horror who leaves a lasting impression on anyone who encounters him.

The titular villain Granny has a zombie-like appearance and muttering creepy phrases, all while chasing the player – you. It is well-known that Granny kidnaps individuals and forces them to play a game in which they have five days to escape or face certain death.
